What Is a PCS Form?

A Physician Certification Statement (PCS) form is a required medical document used to authorize non-emergency ambulance transport for patients whose medical condition prevents them from safely traveling by car, wheelchair van, or other transportation. The PCS confirms that ambulance transport is medically necessary and appropriate for the patient’s condition.

When Is a PCS Form Needed?

A PCS form must be completed before most scheduled ambulance transports, including:

  • Interfacility transfers

  • Hospital or skilled nursing facility discharges

  • Dialysis appointments billed to insurance

  • Specialty care or long-distance transports

The form must be signed by an approved medical professional such as a physician, nurse practitioner, physician assistant, or discharge planner. PCS forms also ensure compliance with Medicare and insurance requirements while protecting the patient, the facility, and the ambulance provider.
